Prohibition of Race and Sex Discrimination - Complaint


This form is to be completed if a student, parent, teacher, school staff member, or members of the public wish to file a complaint alleging a violation of the provisions of 70 O.S. Section 24-157 or Rule 210: 10-1-23.

This form must be completed in its entirety. The complainant will receive notification within ten (10) days of receipt informing them that the complaint has been received and whether it will be investigated. This form will be directed to Mark Officer, Assistant Superintendent of Teaching and Learning.
