Change Forms in Your OPS Employee Portal - Current Employees

To change one of the following forms:

  • Federal W-4
  • State W-4
  • Direct Deposit
  • Name/Address and/or Phone Number Change
  1. Log into the OPS Employee Portal 
  2. Select the Forms/Documents tab
  3. Choose the form you need

Find grievance forms for reporting concerns and following established district procedures for resolution.

Use these forms to request donated sick leave for Certified or Support staff.

Use these forms to request an Extended Leave of Absence for various circumstances.

Please forward completed forms to the Human Resources Department.
